Purpose: To assess the efficacy of a novel orbital tissue expander (OTE) in treating congenital anophthalmic and microphthalmic infants. Methods: The OTE implant is an inflatable (0.5 to >6cc) silicone rubber globe sliding on a titanium T-shaped bone plate secured to the temporal bone with 1mm titanium screws. In vitro testing was performed to assess injection volume versus diameter measurements to determine consistency between devices, flex fatigue for durability of the implants when compressed, weight change in isotonic saline at 37°C to mimic human body temperature, seal durability by puncturing the globe numerous times while inflating, capacity before rupture to determine the maximum amount of saline it is able to contain, and effective sterilization. Ex-vivo testing was performed for adjustments prior to in vivo study. An OTE was then implanted in five 2-week old kittens (OS only) and inflated in 0.5cc increments. Three control animals received enucleation alone. All 8 animals were followed for 18 weeks and underwent euthanasia for morphological and histopathological analysis. Results: In vitro testing confirmed a <5% diameter variance between different OTEs inflated in 0.5cc increments up to 5.0cc, <5% weight change in isotonic saline at 37°C over 7 weeks, <3% weight change over 14 months in the fatigue tester, and no quantifiable leakage (<1mg) after 65 consecutive 30ga needle punctures. The OTEs were successfully sterilized by autoclave and easily secured in the orbit of postmortem kittens. The in vivo study demonstrated biocompatibility of the implant which stimulates orbital bone growth resulting in almost no difference between the implanted socket and the control eye of the cat. There were no adverse effects in the normal maturation, weight gain, and food intake of the cats. Light microscopy showed no signs of foreign body reaction. Pictures of the implants were obtained by using a shadow-photogrammetry system to compare the explanted OTE with the OD control eye. Conclusion: In vitro and in vivo studies show the implant's potential to safely treat anophthalmic and microphthalmic infants.

Purpose: To non-invasively measure the thickness of the anterior and posterior lens capsule, and to determine if it significantly changes during accommodation. Methods: Anterior and posterior capsule thickness was measured on post-mortem lenses using a non-contact optical system using a focus-detection technique. The optical system uses a 670nm laser beam delivered to a single-mode fiber coupler. The output of the fiber coupler is focused on the tissue surface using an aspheric lens (NA=0.68) mounted on a translation stage with a motorized actuator. Light reflected from the sample surface is collected by the fiber coupler and sent to a photoreceiver connected to a computer-controlled data acquisition system. Optical intensity peaks are detected when the aspheric lens is focused on the capsule boundaries. The capsule thickness is equal to the distance traveled between two peaks multiplied by the capsule refractive index. Anterior and posterior lens capsule thickness measurements were performed on 18 cynomolgus (age average: 6±1 years, range: 4-7 years) eyes, 1 rhesus (age: 2 years) eye, and 12 human (age average: 65±16, range: 47-92) eyes during simulated accommodation. The mounted sample was placed under the focusing objective of the optical system so that the light was incident on the center pole. Measurements were taken of the anterior lens capsule in the unstretched and the stretched 5mm states. The lens was flipped, and the same procedure was performed for the posterior lens capsule. Results: The precision of the optical system was determined to be ±0.5um. The resolution is 4um and the sensitivity is 52dB. The human anterior lens capsule thickness was 6.0±1.2um unstretched and 4.9±0.9um stretched (p=0.008). The human posterior lens capsule was 5.7±1.2um unstretched and 5.7±1.4um stretched (p=0.974). The monkey anterior lens capsule thickness was 5.9±1.9um unstretched and 4.8±1.0um stretched (p=0.002). The monkey posterior lens capsule was 5.9±2.0um unstretched and 5.1±1.3um stretched (p=0.128). Conclusions: The results indicate that the primate anterior lens capsule thickness changes during accommodation.

Purpose. Experimentally find a method to provide a safe surgical technique and an inexpensive and long lasting mesoplant for the restoration of vision in patients with bilateral corneal blindness due to ocular surface and stromal diseases. Methods. Identify the least invasive and the safest surgical technique for synthetic cornea implantation. Identify the most compatible biomaterials and the optimal shape a synthetic cornea must have to last a long time when implanted in vivo. Results. Penetrating procedures were deemed too invasive, time consuming, difficult and prone to long term complications. Therefore a non-penetrating delamination technique with central trephination was developed to preserve the integrity of Descemet's membrane and the anterior segment. Even though this approach limits the number of indications, it is acceptable since the majority of patients only have opacities in the stroma. The prosthesis was designed to fit in the removed tissue plane with its skirt fitted under the delaminated stroma. To improve retention, the trephination wall was made conical with the smallest opening on the anterior surface and a hat-shaped mesoplant was made to fit. The skirt was perforated in its perimeter to allow passage of nutrients and tissues ingrowths. To simplify the fabrication procedure, the haptic and optic were made of the same polymer. The intrastromal biocompatibility of several hydrogels was found superior to current clinically used PMMA and PTFE materials. Monobloc mesoplants made of 4 different materials were implanted in rabbits and followed weekly until extrusion occurred. Some remained optically clear allowing for fundus photography. Conclusions. Hydrogel synthetic corneas can be made to survive for periods longer than 1 year. ArF excimer laser photoablation studies are needed to determine the refractive correction potential of these mesoplants. A pilot FDA clinical trial is needed to assess the mesoplant efficacy and very long-term stability.

Ocular tissues are mainly made up from conjunctive fibers on the basis of collagen. The way they are arranged as well as the proportion of water contained in the tissues determines their transparency. As example the fibers in the sclera have diameters between 30 and 300 nm and are arranged in ribbon-like fiber bundles (15 microns thick and 140 to 150 microns wide) that cross each other in all directions [1]. Both different diameters and interlaced structure lead to a tissue displaying scattering properties. By opposition the cornea scatters visible light in a much lower amount, mainly because it is up from fibers having diameters varying only between 19 and 34 microns and that are organized in a parallel fashion within layers. Such a structure is of primary importance when objects have to be imaged through such tissues because scattering results in a blur of the image obtained. Therefore, any irregular change in the structure due to anarchical growth or in diameter as it may be the case with ageing of the crystalline lens results in a loss of visual acuity. The use of biocompatible materials has tremendously increased in ophthalmology during the last years. PMMA still represents the majority of the intraocular lenses replacing the refractive power of the crystalline lens. Viscoelastic materials are currently utilized in the anterior chamber and perfluorocarbons or other polymers like polydimethylsiloxanes (PDMS) are used as vitreous substitutes. The physical properties of such materials should be similar to those of the tissue they replace. Consequently, whenever possible flexible materials that mimic ocular tissues are developed in order to minimize local stress and tissue deformation. Among them, gels that consist of a network crosslinked in a balanced salt solution (BSS) have also been tested as intracorneal implants for GIAK (gel injection adjustable keratoplasty), a technique designed to correct myopia [2]. Because the number of intermolecular bonds per linear chain governs the rigidity of the material, various collagenous tissues can be mimicked by controlling both concentration of the network and the degree of crosslinking. However, when such materials are implanted within the optical path of the eye, their optical properties should be scrutinized. A high transparency is necessary in the visible part of the spectrum (450-700nm), associated with a high absorption in the UV and the deep blue part (400-450nm) for protection of the retina and the crystalline lens. In addition, scattering should also be minimized to avoid loss of visual acuity. Therefore, apart from an accurate shape and refractive index, the surface quality and the homogeneity of the implants have also to be as high as possible. The purpose of this study was to compare the scattering of such materials to that of ocular structures.

Two centuries ago, Pellier de Quengsy suggested the implantation of a glass plate in the cornea, and the Chevalier de Tadini the replacement of the crystalline with a glass lens. This was the beginning of ophthalmic prostheses. Since, material sciences have made tremendous progress with respect to ocular implantation. As postoperative implant rejection and complications have to be minimized, several requirements on the materials to be selected can be given: In order to minimize stress locations and local tissue deformation, the physical properties of the implants should be similar to those of the replaced tissue. The material should remain stable in time and not degrade and, in some instance, they should be colonizable. The implants should not induce reactions to surrounding tissues, and therefore should be non-carcinogenic, nonallergenic and non-immunogenic. When the material is implanted within the optical path, its optical properties should be similar to those of the replaced tissue. Especially, a high transparency is required in the visible part of the spectrum (400-700 nm) with a low scattering. As the retina and the lens need to be protected from UV radiations and, to a lesser extend, from radiations in the deep-blue, the materials should also present a high degree of absorption at those wavelengths. To avoid potential complications at long term, an absorption as large as that of the eye tissues should also exist in the infrared part of the spectrum. Because light has to be focused sharply on the retina, the implant geometry and index of refraction have to accurately selected. In addition, the surface quality and the homogeneity of the implants have to be as high as possible. Ideally, to respect the natural physiology of the eye, the materials' index of refraction should match that of the tissue it is design to replace. Consequently, whenever possible the implant material should mimic the properties of ocular tissues. Certain glasses and crystals (sapphire) mounted on metal alloys or ceramics fixation devices (haptic), have been used for the fabrication of intraocular lenses and keratoprostheses. However, the most well-known material for ophthalmic implantation is the thermoplastic polymer PMMA (Polymethylmethacrylate) obtained by polymerization of its monomer, methylmethacrylate. Its ocular stability was accidentally demonstrated after the second world war [1]. Ridley took first advantages of this material for intraocular lenses [2] and, a few years later, MacPherson and Anderson for intracomeal implants [3]. Synthetic polymers are made of very large molecules consisting of either three-dimension (bulk) or two-dimension (linear chains) crosslinked repeating monomers. Thermoplastics are organic polymers having in common a long repetitive carbon chain [-C-]. The mechanical properties of these polymers depend principally on the van der Waals intermolecular forces. In some other polymers the various chains are chemically linked to each other by covalent bonds. Carbon, however, is not the only atom possessing the ability to form very large molecule with other atoms. Silicon is another element that exhibits similar properties. The chain is made in this case of repetitive siloxane units [-Si-O-], whereby the molecular weight of the polymer is determined by the chain's length and type of cross-linking. At low molecular weight, a few thousands centistokes (ctsk), and end-crosslinking, polymers are fluid. Because of their viscosity and their ability to repel water, they are often referred to as silicone oils [4]. Gels are another form of matter, an intermediate between solid and liquid, that can also be used for the fabrication of implants. Gels consist of long chain molecules (generally polymers) that are bulk cross-linked to create a topological complex network immersed in a liquid medium. The latter prevents the polymer network for collapsing while the former traps the liquid. Such gels have been found to be very sensitive to external parameters such as temperature, pH, pressure and salt concentration. The rigidity of a gel is function of the number of crosslinks per linear chain composing the bulk. Most ocular tissues, such as the vitreous, a loose low rigidity network made of proteins (<99 % H20), and the corneal stroma (or the sciera), a dense network composed of collagen (<75%H20),could be considered as gels.
